What is that black on-board 9-pin connector for?

That’s is 9-pin USB port. It’s used to connect the PCE-AC58BT card to a 9-pin USB port on motherboard to use it’s bluetooth function. A 9-pin to 9-pin USB adapter cable is included in its package. Without this connection, the card is only used for Wifi. Your motherboard should have a available 9-pin USB to connect. I don’t know if you can use a 9-pin USB to male USB A adapter cable to hook up the card to outside USB A port on your computer in case your mother board doesn’t have a 9-pin USB port available.

Can I use this with my headphone with mic?

I agree with James T. It should work just fine as long as you have the bluetooth driver installed. I am assuming that the headphone does have bluetooth feature.

Is the low profile bracket included?

Yes, but don't buy this.  It doesn't work.  Wi-Fi won't work. BT is about it for functionality with Win10.

Is the NGFF / M.2 slot on the PCIe card actually Key A ? Having a difficult time finding an adapter to use a Key A type wifi adapter card.

The original card in it was a intel 9260. I replaced with a ax210 which shows A & E and it fit in just fine.
