Are phase II rewards the current rewards displayed via the www AMD com / radeonrewards link, or will a different set of games be offered ?

Yes. I just redeemed my Silver Tier reward last night (Feb 11) and those 5 games were available. Note that the game Thief is not released yet. I chose Thief and they will send me a code for the game once it is released (Feb 25 I think). The Silver game reward choices seem to be Thief, Deus Ex: Human Revolution, Hitman: Absolution, Dirt 3, and Sleeping Dogs. (As of today, Feb 12)

Is the game thief a 5$ type pre-order or a full costing game pre-order?

Depends on where or how you are going to purchase , I got the Radeon rewards level 2 and I picked Theif and dirt 3 as a free game. I received a cd key for steam for dirt 3 and when thief comes out I will get a cd key for theif. If I were to purchase their from steam you would have to pay the full cost of the game less the pre order discount. If you are going to go to game stop it is put down a payment and pay the remainder upon pick up.

Question 3

Less of a question than an observation. It's not specifically stated on the product page how you will receive this. I'm guessing since I neither saw a pop-up webpage with the code nor received an e-mail with the code to redeem for my free games, that the code will arrive on a paper shipped with the graphics card. Am I correct?

You receive a card that looks like a post card when you receive your package. It has a scratch code.
