Yes, it has both connections, it also has the power connector for pata.

Does this work on windows 8.1?

Worked in windows 7 without additional drivers, should work with win8.1. It is a standard usb to sata/pata bridge device, windows has a standard driver for it.

Does it include LED lights?  For example an LED light showing it is plugged into USB and recognized.  Also whether a SATA drive is connected etc.

It has an LED light on the power supply, but I do not remember whether what looks like an LED on the adapter actually lights up.  That dot that looks like it might be an LED is located by the S (SATA)--I (IDE) switch.  My unit worked very well for what I needed and has cables for all the possibilities.  I don't have a spare drive handy to use to refresh my memory.  The operating system recognized the drive when I did use it.

I have a 2002 WD hard drive (Enhanced IDE) that I grabbed from the basement. It likely has scans of paperwork that I need today.

Yes Would let you copy everything off the hard drive as long as hard drive still works.

Can someone provide the USB Bridge Controller for this device? I have had issues using this 'type' of device with smartmontools in Parted Magic.

What is the data speed of copying and writing using this cable?

I tested it with Crystal Disk Mark on some some SATA & IDE hard drives and got the same speeds when plugged into a USB 3.0 port as when I had them plugged into the motherboard.

Don't expect miracles from USB 2.  You're only going to get USB 2 speeds.
