Question 1

I was told that this would be able to go from my 15 pin cable to a USB port.
One end needs to be female 15 pin and the other a USB male end.
But looking at the picture is looks like it has 15 pins on both sides.
Is the picture incorrect?
If so is there a cable that would work for me?

The picture is accurate. This does not come with a USB end. I have heard of such adapters as adapters that go from VGA to Ethernet. I imagine you could probably search for them on Newegg.

Can this cable be used effectively with higher resolutions?

Sure can be used for higher resolution.  Been watchin' movies on my UHD TV no problem with this here cable.

Does a regular installation of this cable require male to male, or male to female?

Male to PC and male to Monitor is the regular installation.  If you needed an extension, then one end needs to female.
