No they do not have a headphone jack. I would recommend purchasing another type of unit that connects to your TV or surround sound unit. There are some decent priced wireless headphone units on Newegg that work well with this player.

Is this a refurbished item?

When I purchased mines, it was not refurbished; it was a part of a bundle deal with the Total Defense Internet Security (new) and I called New Egg just to be sure. This one appears to be new. If it was refurbished it would let you know in the product description. Hope I answered your question.

Question 3

I noticed the model number is 3100R - does that mean it is refurbished?  I don't see it in the listing but on other listings the R seems to indicate refurbished.

Er - I don't know that I can conclusively say that these are not refurbished. What I can tell you is that I ordered my bundle January 3, 2014, and nothing in the description then - as now - mentions refurbished. Nor did any of the confirmation emails I received from New Egg. I can also tell you that at arrival, nothing in the actual packaging of the Rokus indicated refurbished - which, I believe there would have to be a sticker or other indication indicating that they had been refurbished. I can also tell you that the 2 players have been working beautifully!  If pressed, I'd would say that they are new for lack of any indication to the contrary, but I haven't really researched your model number question.  Good luck!  It's a heck of a deal if you ask me. You wouldn't be disappointed if you're looking for 2!

1.  Is this a new product, or re-furbished
2.  Does it support m4v video files also?  
I already own one ROKU and want to buy a second, but want to make sure it plays M4V video files also just like my first one

MP4 and ASF/WMV files encoded in VC-1 can be displayed at a maximum resolution of 1080p/this is copied from roku support site for roku2 xs /mine looks to be new???sealed w/plastic film on surface of unit works great...

Is it possible to read a samba or cifs share off a local network to play mp4's on this device?  If not, is it possible to utilize a local Windows Media or Itunes server to play mp4's and music through this device?

You can setup a Plex server for free if you were willing to research it.
Plex will allow you to stream local network content but I would verify with Roku2. I have a 3 and it works.
