does unit have HDMI ports, can it be used for video surveillance monitoring?

No, unfortunately there is no HDMI port on this monitor. I purchased this for a family member after their original monitor died. This monitor does have DVI and VGA, you can purchase a DVI to HDMI cable and connect the DVI into the monitor and the HDMI into whatever you plan on hooking into and that should do the trick. The monitor will not be HD or Digital however I believe, it will still be analog. I hope this helps... if you check the picture of the back you can see the 2 inputs the monitor offers.

Yes, all was included, and yes the picture quality is what you would expect from the specs of the monitor.

Yes, it works just fine under windows XP. Keep in mind it is a square screen, not wide screen format like the more common monitors being sold these days.
