This model (1350M) comes with the actual battery pack installed, correct?  The APC website says "included battery modules, 0" in the specifications

Yes, it comes with the battery installed, but is flipped around and has to be removed and inserted correctly before using.

What battery does this use ?

Since I just got this UPS, I do not anticipate needing a new battery in the next 2-3 years. There are at least 2 ways to find the correct battery. 1st, open the battery case and look at the battery that is installed. Second, contact APC site and see which battery they recommend. If you just want a 12 volt rechargeable battery, check with NEWEGG or some other source that can supply a battery of the size you require. There are several different batteries used by APC, based on the reserve rating of their units.

Is a PowerChute CD/DVD included ?

The PowerChute software is downloaded from the APC web site. Instructions are inside the user manual, which is included with the BackUPS unit.

What size UPS do I need to handle a 8 year old Panasonic 50" Plasma TV?

the BX1350M should do just fine, this model will also allow you to turn off the audible alert without connecting it to a PC running the power chute software. where as the smaller ones you would get at wally world do not have that feature.
