Question 1

I see from the picture you would ideally put it under your desk right in front of you. How does it exactly stay attached to the desk? Mine is a flat surface so Im not sure how it would attach.

The rails mount underneath the desktop by screwing into it from underneath.

Note that some desks have hollow cores and may have only a thin veneer on the underside. My Ikea desk with a  glossy white finish falls into this category. In those circumstances, you have to be careful not strip the wood out when tightening the screws.

If this a concern, you can also attach the four rail legs using industrial-strength Velcro that you can buy at Home Depot or a convenience store. This only works if you have a smooth surface like a plastic gloss veneer. If you're concerned about screwing into and damaging your desk, you can try this first...it may peel off but I've seen it hold well even if you lean on the keyboard.

Question 2

Does the drawer rise to the height of the desk when  it is pulled out?

Hello ...No, it it drops down to just above the knees of a tall to medium height person.  Pretty sturdy, decent (not great but enough) for the mouse, has been with fairly heavy use at my office for about 3 weeks, it works well for us...with our conventional desk.

Can the keyboard tray retract completely under the desk?

Yes. It all depends on how far back you attach the mounts. I have two, side by side, and mounted them so only the end of the curved edges protrudes from the front of the desk. This make it easy to see where they are and pull them open. If I were to mount them another inch back they would be completely under the desktop. The still pull out plenty far to expose the whole keyboard.
This is a great, inexpensive keyboard drawer. Most cost too much for me. But it is largely plastic so you can't abuse it or lean on it much. If you are careful it should serve you for a long time. BTW, I removed the side mouse attachment and have the mouse on my desk. It's easier to use that way for me, and eliminated resting my hand / arm on the keyboard tray, which could shorten its life if you're not careful.
I recommend this keyboard tray, but be kind to it and it should be kind to you!

I have an opening under my desk of 21 1/2 inch width. My current keyboard tray is exactly 21 inches without hardware. Will this item fit the width under my desk?

Hi Debbie,
This keyboard tray is 21" wide. However, the mounting tabs take up just over an additional inch on each side. So, the total width needed to mount it is nearly 23 1/2".
I have two of these mounted side by side. Because of this I had to remove the pull-out 'side car' where a mouse would be used. This is not a problem for me as having the mouse (mice) on the desktop is not a problem.
I hope you have the space to mount this tray. It is the best thing going for the price and actually pretty solid, but don't lean down or bang on it. The mounting tabs and slide out rails are metal but the tray itself is plastic. Still, I am very happy with what I got for what I paid.

Currently I have Tennis Elbow so I need to the Keyboard and mouse as close to my lap as possible, how low can this be adjusted?

It really doesn't adjust down to your lap. Unfortunately if you want something like that you simply have to pay the extra money for a big arm that will adjust your keyboard and mouse to whatever level/position you need. This is an inexpensive keyboard drawer that was nice and easy to install, but doesn't have the feature you're looking for.

It may go lower, but not as stable as I would like.  So consider 3-1/4" as the lowest setting it goes.  It does fasten under the desk.
