Do i need to buy , Hard drive and ram ?

Yes, you do. It's a barebones PC, meaning that it's not complete. You will need to buy RAM and either a hard drive or SSD. The hard drive will have to be a 2.5" laptop drive for it to fit. The SSD however can either be a 2.5" or an mSATA.

You will also need an OS, since it doesn't have a hard drive or SSD for it to be installed on. And don't forget that the RAM has to be 1.35v. Hopefully this answered your question fully.

How good is a R9 m275x for gaming?

It depends what kind of games you would like to play at what resolution. I have tried Battlefield 4 (FPS) on it. Everything is set to low, the textures set to medium at 1280x720 resolution with v-sync, I was getting 40-60 fps. Skyrim (First person RPG) defaults to high graphics and is playable at those settings, but would recommend to lower them at higher resolutions. Last game I've tried recently on it is Supreme Commander (RTS). I have it set to highest graphics and it handles the game fine, even with maxed out units. It does dramatically jump FPS when zooming in and out of the map, but levels off usually in a few seconds. In conclusion: Good GPU, just, lacking a bit in the CPU area. Also make sure you have it set so the games use the "High Performance" GPU instead of the "Power Saving" one. It does make a huge difference.

Would replacing the OEM fans with more expensive fans help the noise and cooling issues?

I'm sure the noise would decrease if you put quieter fans that could move the same amount of air. You would have to find some fans that don't have the standard connection 4 pin connection for a chassis fan, they are the 4 pin for most GPU fans. If you want to see how the heatsinks are set up, google "Brix 8890 teardown". Essentially, there are 2 boards connected to each other with a bridge, the gpu is on the bottom board and the cpu is on the top board. The two heat sinks are sandwiched in between the two boards. So, yes, more air will be better, but, it won't be too dramatic of a difference.
