The description says "The Last of Us Remastered voucher".  Does that mean I'll need to order the game separately or does the game come with the PS4?

The voucher is a code that you enter into the PlayStation Store (an app accessed on the PS4) that allows you to download the game for free. In a sense it's a separate affair because the disc isn't included, but you get the free download code with the PS4.
