Are the filter and the light bulb included?

Yes, the filter is included. 
No, the light bulb is not included, but the hood takes a regular base light bulb that you would use in a lamp or light fixture. I use a 13W compact CFL.

Is this a wall mount or under cabinet mount?

I used it as an under the counter mount.  
I have been happy with it so far.  The price was lower than Home Depot or Lowes.

does it come with a plug?

No, you have to do a little electrical wiring. Considering that I have absolutely no knowledge in electrical wiring, it was still easy to do. Just need to turn off the power in the kitchen so you don't get zapped, then just twist the right wires together and cover with that protective plastic twist cap. Directions are easy to follow but I think I just looked at my old range hood's wiring and copied it.
