Is this item Windows 8 compatible?

I can't say it is compatible with Windows 8, don't have it, but it is compatible with Windows XP and Windows 7 as I use it on both OS's.

Whats the output voltage on the serial side?

That is an interesting question. Regular RS232 voltages range from -13 to +13 volts DC. The maximum out put of a USB port is 5 volts DC. I do not know the exact answer, but volts DC 5 volts or less. I have bout about 50 of these devices and they work well driving older serial based equipment.
