seeking clarification; unclear based on image whether this item includes the wire that goes from the earpeice to the phone itself including the jack?

Yes, you do have a wire to deal with. I put the phone in my pocket and walk around hands free. You just have to watch not to catch the wire on something. The range is terrific too so I can go far outside with it. the sound quality is great tool. I love it!!!
