What is class 4 mean and is this product new with new box?

The Class is something to do with the quality, depending on what you want to use it for you need a higher class. I use mine for a digital picture frame and it is fine. Yes it is brand new in the package.

Is the adapter included?

The adapter to make it a larger sd card is included, but not the usb adapter that you would plug it into if your computer doesn't have the slot.
