Does this have a Reference of Custom PCB? I want to put a Water-block on it.

The pcb is only half the size of the card, the back fan blows through a fin stack that is open on the back. It might be a reference nano pcb, it's definitely not a normal reference pcb.

Would this card fit in an NZXT H400 case?

Yes, the card will fit. However if you have a front mounted radiator, especially if it's a 50mm (2") thick radiator, you may have to relocate the radiator to the top of the case. I have mine installed in a Fractal Design Meshify C which has similar dimensions, and even if I had a standard 1" thick radiator in front the card should just clear it. And you'll need about 1" of clearance above the GPU because the PCI-E cable need to connect to the middle of the card from the top, but you should have room.

I've recently purchased one and so far I'm happy with the purchase for the price. However, I will compare it to a Sapphire reference cooler vega 56 so that you can make the decision for yourself. Keep in mind that I have flashed both cards to use the vega 64 bios, and have used the exact same settings to over clock / under volt these cards for mining ethereum.

- The Sapphire pulse is a thicker card. Over 1/4" thicker than the reference cooler due to the beefy heat sink and double fan design.
- The Sapphire pulse at full load uses about 10W less power than the reference cooler design.
- The Sapphire pulse runs 2 fans at 20% speed vs the reference cooler using 1 fan at 40% speed to keep the card @ around 66C, 70C on the hot spot, and 80C on the HBM. 
- The Sapphire pulse is INFINITELY quieter than it's reference cooler counterpart. 
- The HBM on the pulse is running a bit warmer at a steady 82C instead of 78-81C fluctuation that I see on the reference cooler version.k
- The Sapphire pulse DOES NOT have the LED indicators for how much work the card is doing like the reference cooler design. This is a bit of a bummer, as it comes in very handy at quickly and visually identifying if a card has locked up on you. 
- The Sapphire pulse DOES have the bios switch just like the reference cooler design, but it's a bit of a pain to access it with a finger. I had to use an object to effectively use the switch.
- The pulse card came with dependable Samsung memory that allowed the card to over clock to the performance I wanted, UNLIKE the XFX dual fan version which was completely unstable with marginal over clocking. I would NOT recommend the XFX card. It was more expensive and a disappointment.

I'd say the low cost and lack of noise make this card a serious contender to the reference cooler version. I would buy another one if these guys didn't limit me to ONE!
