Question 1

Does the card allow speeds higher than the stated "6Gbps Data Transfer Rate"?  Is that the SATA drive speed?  An M.2 NVMe in a 4x PCIe slot should be faster.

My 960 PRO has the exact same speeds weather directly inside the X99 Sabertooth m.2 slot or in the Expansion cards m.2 slot. My purchase was to reduce heat while adding small heatsinks, which this allowed me to perfectly. Just be sure that your MB allows you to set it to 4x, It never auto-detected for me correctly.

Could I use this with an ASUS 970 PRO GAMING/AURA and a WDS100T2B0B  WD Blue 3D NAND 1TB SATA III M.2 2280 Solid State Drive to make the M.2 SSD a Bootable Drive?

The way it is design it actually can be used as a m.2 and as a SATA ssd.  Considering you are using an older amd chipset I don't think it supports m.2 so you would have to do like I did and attach a SATA cable to the card and run it as a SATA ssd.  You will have to go into windows device manager and get the computer to recognize the drive.  Format the drive then you are good to go.

Question 3

To make sure, this has one slot that runs at NVMe speeds, and one slot that runs at SATA 3.0 speeds?

Yes, and actually you cannot put SATA stick into NVME port, it won't work. You have to use SATA slot and use SATA cable to connect it to motherboard. 
NVME drive will work fine.
