only for activation and installation. Every once in a while steam can require a check on your account information and require the internet, but other then that Steam and its games are fully playable offline unless otherwise stated (i.e. multiplayer game)

Yes it requires steam, I don't believe that the content it region locked though. As for requiring internet access, no, however you will need it the first time you launch it.
