Hello,
What does the Motherboard header connector provide, I don't see this listed on many similar cards; does this card include the cable to connect to the motherboard?

It is used for power/ data connections for internal card reader ( for CF, SD ,etc ) hookups and other such devices.

No, a cable doesn't come with it,  this is attached to the card reader device I used. It is possibly on other USB devices as well.

It just attaches unto the Motherboard header connector pins on the card.

For a 400mm (15.748 inches) M-F extension cable that splices between the male card pins and the Female cable from the card reader try to Google  "EXUSBI-40" (without the quotes of course.)

will the jp1 work as a added usb port for a card reader? and is this windows 10 compatible?

Yes, this should add several USB 2.0 ports for you to plugin a USB Card Reader.  If the card reader is external, you plugin to the ports on the rear plate.  If the card reader is internal, like in a 3.5" drive external drive bay or in a 5.25" external drive bay, both on the front of the PC, then you may have some work to do to get the proper cable to go from the card to the card reader.

As for Windows 10, I do not know.  I am guessing that you'll be fine with 10 assuming that Microsoft retained the basic drivers for the normal USB 2.0 chips.

This card uses an NEC chip, if I recall correctly, and I have had no problem with drivers for it in either Windows, up through version 7, nor in Linux.  So with Windows 10, you *MAY* have to download and install a driver *OR* Microsoft may have been "nice" and provided the NEC chip driver in the Windows 10 kernel.  You just need to buy the card, plug it in, and see what you get with Win 10 *without* installing any drivers FIRST. Just plug in the card and boot up 10.  If Windows 10 detects the card, finds teh driver, and installs the driver for it automatically, which is likely, then you're done.

If Windows 10 has an HCL,   a Hardware Compatability List,  then you can do a Google Search for  "Windows 10 HCL"   and see if this NEC chip (IIRC) is on the list.   It is failry normal for an OS, like Windows or Linux, to have an HCL that specifies which devices and which chips are supported in the Kernal.

Does the card come with a Driver CD?

No driver required.  It is plug and play.  It does require an open 9-pin connection on your motherboard to connect the card to the onboard USB system though, which you will see in the picture of the card on the NewEgg website.

I bought it for Linux to enable a CrystalFontz LCD display in MythTV. The PC I am using did not include any IDC header ports on the motherboard so this was perfect. Plug and play no drivers needed.

I've had serious compatibility problems with PCI USB adapters on Win7 Home Premium 32-bit. No drivers were available to fix problems. Will this card work on Win7 32-bit?

I tried to use the 2 port and the 4 port version of this Startech board and could not get either one to work. Even called tech support. Got the 2 port one to work for a couple of days and then it stopped working. Never could get it to work again. Sent both boards back for refund.  Then purchased the SYBA Model SY-PEX20135. This board uses the Etron Chipset. Driver installed with no problems and the board has functioned flawlessly since I installed it. Recommend you install this board.

If it worked in your computer with either 7 or 8 or 8.1 it will work with 10 as well.  So yes it will work with Windows 10, as long as you have an available PCI slot to plug it into !!
