Does this splitter only duplicate the display from the source to the other monitors or will it allow the two others monitors to become an extension of the first?

This is a splitter only. Most HDMI outputs would not support dual output at 4k. If you have a Displayport or mini displayport on your computer that can handle the bandwidth requirements for sure, but the video card in a laptop or older desktop may not be powerful enough to support two 4k displays. Ilford instance a windows surface pro 2 had trouble running dual output to 1080P screens but my GTX980 runs two 4k monitors well to watch a 4k movie and brows the web simultaneously, but not game at decent frame rates while watching a 4k movie.

Will this device allow you to extend the image between two monitors?

not sure what you mean by extend..The unit will display one single video source on 2 separate monitors the video stream can be as large as 2160P@30hz.
