Does this also transfer audio, for watching movies played on the laptop to a TV?

No it will not, as the VGA side as a standard was not engineered to carry sound. I used this to drive a VGA input Monitor. Best!

I want to use a flat screen monitor with an rs232 cable as a second screen for my laptop. The laptop only has a HDMI slot. Does this cable provide the interface?

I think you may be mistaken about the cable. RS232 is a 9 pin connector( 2 rows of pins). RS232 cables are not used for video connection.

The adapter is to convert from the HDMI receptacle on your computer to a VGA port which is a 15 pin connection (3 rows of 5 pins). You will need a VGA cable, male to male to connect from this device to your monitor. (male means that there a pins) Male to male means that both ends of the cable have pins.

I honestly don't know as I have not encountered that situation.  
the issue would be is if the PC you are connecting to having the VGA would have to be male and most are female on the PC.  You would have to check to see if the ends could meet.  If they do, but doubtful, it should work.

Does this actually convert HDMI signal VGA signal or is it just a pin-out adapter?

this does convert the information, though you will not be able to use all the functions like an HDMI would (sound, HI DEF resolutions, etc...) all it really does is changes digital information to analog information. that is only the video part of course
