Question 1

Where do you clip your wrist strap?  I see a lot of mats where the wrist strap is already connected to the ground strap.  Can you just clip your wrist strap to the mat?

This particular mat only has a single snap to connect the mat itself to ground. You must connect the included wrist strap to a common ground independently.

For example, you could connect both the mat and the wrist strap to the grounding pin of the frame's power connector.

If you connect the mat & the strap to different grounding points (e.g., grounding pin & metal part of the frame) it's always a good idea to use an ohm meter to make absolutely sure that the grounds are really common.

In any case, the grounding pin is usually your best bet.

What is it supposed to clip onto? The computer? Wrist Strap? My belt?

The clip goes onto the case. Its used to draw any static electricity from the case to the ESD mat and prevent any sort of shocking of the components of your computer. I reccomend for some extra safety also purchasing a ESD wrist strap that makes handling components safer when readying them to be put in the case. They usually range anywhere from 5-10$ and can save you lots of time and hassle. ESD wrist straps go right on ur wrist and also connect to the case.

Question 3

Has anyone bought this to use on a desk to prevent ESD to keyboard and mouse?   Every time I walk 3 ft or more on my carpet, I become statically charged and usually end up discharging through my mouse which causes the USB connection to flicker.  I am afraid it is killing my mouse and possibly the usb ports on my PC.    I was planning to put this mat on my desk and attach the cable to earth ground so I can touch the mat before touching my mouse to discharge myself.

Honestly, touchibg anything metal such as your computer case, part of your chair, or desk would gove you the same results.  This mat is rather large and depending on what computer mouse you use would prevent the laser from tracking.

Over all this doesnt sound like a horrible idea for your situation.

A few things to consider... You may want to look into anti static floor mats first as that may be less intruive.  They even make electo static discargers which attach to your shoes.  Oh also, humidity helps to prevent electro static form building up(obviously dont over do it).

This Anti-Stat Mat has a snap for the wrist strap only and not for the ground strap with a larger snap. You can actually buy a snap adaptor that has both snap connections ( Pomona 5532 Stacking snap adaptor) for $3.95 each.

This has the 8mm snap built in not the bigger 10mm for the ground strap. We opened one of the box and checked it out.
