Will this extend the display across 3 monitors or just duplicate?

It will display as both extended and duplicate, provided that your graphics adapter will support the configuration. There is one drawback. On the AMD catalyst control center, the settings do not persist which means that I have to reconfigure the desktop layout every time I log in -- which is a bit of a pain -- but it works fine once the multidisplays are set up. However, I expect this is a tempory problem. The only thing I don't like -- and I am not suggesting that this is a problem associated with the display port, but it bugs me that there isn't a panel aross the top of each monitor that displays just the files /folders/apps that are open on the screen. As s, when you have a lot of things open, you can't read the tabs because they are all truncated. Another thing I hope that gets rectified.

Question 2

Is this backwards compatible? I only have 1 DP connection, on my 4K monitor, but want to have 2 DP inputs for my 1 monitor. Will this work, as I only see the words output on the one side of the DP device.

If I understand your question correctly, this is not the product you are looking for. 
This will take one display port output on say a graphics card and drive two or three separate display port capable monitors.

Question 3

I have a laptop whose graphics card has the basic capability of supporting two monitors, whether it be its screen + DVI monitor, or two DVI monitors (one thru the DVI port and one thru the DisplayPort with a simple, non-active DisplayPort to DVI adapter).

Additionally, I am able to use up to four monitors: its own screen; one DVI monitor through the DVI port; and two more DVI monitors through the two DisplayPorts, using an active DisplayPort to DVI adapter on each of the DisplayPorts.

My question is, will this item run 3 DVI monitors by using a simple non-active DisplayPort to DVI adapter on each of its 3 outputs, and then I could also run the laptop screen, and one DVI monitor off the DVI port? For a total of five monitors?

FYI, all monitors referenced are 1920x1080.

As long as all monitors have the same recommended resolution, you should be okay with that - providing your video card can really handle 3 outputs at the same resolution and refresh rate. In a laptop, I would not be surprised if performance is hindered or if your video card gets really hot.

Will this support 21:9 via displayport when connected to either 2 or 3 ultrawide  monitors?

If your 21:9 monitors are within the maximum resolution stated of 2560x1600.  I will say that my experience with three 27in monitors at 2560x1440 resolution, I wasn't able to have the best luck getting all monitors to stay on using nVidia 780ti GPUs in SLI.  Two worked, three was troublesome and I ended up using the GPU ports.
