Is there an issue with this hard drive that it requires a heatsink?

No, I read a few reviews where the the heatsink got in the way and they removed it without any issues. The heat sink helps keep it cooler, more stable, and possibly extends the life. At the speeds NVME drives run, they all could benefit from a heat sink. Some motherboards even include a heatsink for the M.2. It's a lot like buying ram with or without a heatsink.

Does this part come with a mounting screw?

Mine didn't. The screw should come with MB or the case.
