Does this come with rackmount ears?

Yes - it does have rack "ears" that also work well for wall mounting (turn 90 degrees).  The unit needs 10" of clearance so a 4" or 6" patch panel rack wont work.

How do I hook this up to a wireless cable modem. Modem has 4 ethernet ports?

This is a network switch, which requires a router.  Since your modem has four ethernet ports, it sounds like it might be a combo modem/router.  Check to be certain.  If it does include a router, just run an ethernet cable from one of your modem's LAN ports to the IN port on the switch.  You can then connect up to 24 wired devices to the switch.  If your modem does not have a built-in router, you will need to add one between the modem and this switch.  Good luck!
