The kit comes with two little push-pin screw things that clip into the PCB of the graphics card.  Also in the kit is the thermal paste you apply to the top of the GPU chip.  Then you just screw this heatsink on down over the GPU chip.  There are also little heatsinks for the RAM chips.  The power plug has a three pin small plug and a 12V disk drive power adapter plug to power the fan.
