Do I have to buy s/w or s/w subscription to use this device ?  ALSO I need input & output IP to be the same. All I need it for is IP blocking before I get to my network.

You do not have to buy software or a subscription to use the device. The software is free to download. You can buy a CloudKey if you do not want to run the software on one of your computers.

I am not sure what you mean by the Input and Output IP to be the same. Two ports cannot have the same IP address. So, Wan1 and Lan1 must have different IP addresses. But, I am not sure that is what you are asking.

Question 2

Wrong specs are listed for this device as of 12/08/2017. Specs listed are for UniFi USG not UniFi USG-PRO. Check the vendor website for proper specs, dimensions, etc.  Adding a question mark so this with post..?

You are absolutely correct. The specifications listed for this device is that of the Ubiquiti's UniFi USG and not the USG-PRO.

I get around 940 Mbps / 940 Mbps when testing through my local Gigabit provider, Consolidated Communications.

Does the UBNT USG-PRO-4 Gateway Router support multiple virtual WAN IP's, EG natting and patting to different internal IP's? Any limitations?

Yes. i currently do this. i have one ISP link with 5 statics that I NAT to different internal IPs. I have a different ISP link that i use for general internet. the setup is a little cumbersome, but i've been doing this for a couple years now.

What is the throughput of the router when all security features are enabled?

Specs say less than 250 Mbps when IPS is turned on. IPS is still a beta feature.
