Does this include the DVI/HDMI bracket? The picture shows it but the description does not mention it.

Yes it does. We bought this specifically for that bracket and we were not disappointed.

Does this come with the card to install into a PCIe slot in my motherboard or is it just the VGA attachments?

This item works with any standard low profile card . It has slots for vga, s-video, HDMI, and dvi, but make sure your card is nvidia since its XFX.
