Can you use the 3.5 mm jack to use a wired headset with mic?

You should take a look at the user manual located by searching in google for "Yealink ehs36 manual" It details specific connections. It does have the ability to support 3.5mm, but it may depend on your particular headset. We do not use the 3.5mm option so I can't speak to it directly.

Will this work with a Plantronics blue tooth peice, PLT_M165?

The Yealink EHS36 Adapter does NOT support bluetooth. If you want to use a Bluetooth device with a Yealink phone you will need to buy a Yealink Bluetooth adapter.

Does this come with the 3.5 mm cable to connect the Yealink EHS36 to the base of the wireless headset, the RJ12 cable from the EHS36 to the EXT port and the the RJ11 cable?

You can see the packaging contents by Google searching: Yealink EHS36, look for the User Guide PDF link. The User Guides shows all of the packaging contents of a new EHS36 adapter. And, the adapter you'll buy from Newegg is new, unused, and contains the items included from the manufacturer. But, to answer your question: Yes to all
