Question 1

The system fan headers are where the chassis fan go into correct? I didn't find any fan header cable that came with the board. My case has 3 fans, but the board only has 2 SYS_FAN headers. :(

I might have to spring for a splitter or connect one of the fans directly to the PSU but that would mean it would run at full speed all the time.

If you have an active CPU cooler (e.g. a heatsink with a fan on it), this board basically needs no cooling at all.  One fan over the memory would be more than plenty.  If you're using passive CPU cooling (heatsink, no fan on top) you probably need to run all the fans at full blast anyhow.

Question 2

Does this board support Haswell Refresh CPUs or will its BIOS need to be updated before using Haswell Refresh Pentium G3240 CPU?

I was able to install a Pentium G3258 (Haswell refresh) with the default BIOS. I did not have to update anything for the CPU to be recognized by the motherboard.

Does this ship with a BIOS that is compatible with Haswell Refresh CPUs or would you have to update the BIOS first before using the board?

Mine worked fine with a Haswell, but I wouldn't guarantee that would work for any board shipped - it's too hard to know how old stock is.
