can games be played on it?

Yes, it can run any Android app which is compatible including most games.  It runs gingerbread 2.3.5 and has limited CPU speed and memory, so fancy 3d programs might be choppy or not work if they need a fast gpu, but flat puzzle games work, you can play videos, and browse the web, and everything else I do on my 10" tablet except it fits in my pocket and the screen is smaller.  But the ui is tuned to have nice big buttons so in some ways it is better.  It will accept a 64gB sdxc if you format it is fat32.  I've had one for two years and it is my main media device.

does it show visible use wear or scratches?

If you're asking about the condition of the rebuilt player as I received it, mine was like new.  After six months use, It's not unusually fragile but I've managed to put some minor scratches on mine in the last few months.
I did have to send it back to Samsung for guarantee covered repairs.  I think they replaced the battery, but don't know of sure. (they didn't say)  It took a while and was a minor inconvenience,  but overall I'm relatively happy with the purchase.

Does this come with headphones?

Yes, mine came with a pair of white/Samsung earbuds look up item 9SIA2020ZV4643 here on Newegg for photo.
However it came with the wrong charging cable and email to Newegg support was answered with please contact the supplier.
