Where are the outlets? The holes on top of it don't look like outlets?

I know what you're saying - everyone seems to be using stock images that look more like renderings than actual photos, but in real life, it actually does have 12 outlets.  The outlets have sliding covers, presumably to protect small children.

There are six such down the center, similar to a traditional power strip, and then three more along each edge which are rotated 90 degrees from the center outlets.

I bought it for an application where everything that needed to plug in had a power brick, where the physical size and orientation of the outlets made it work well.  The built-in wire management feature was mildly useful to me as well.  The slightly longer than usual cord avoided the need for an extension cord in my installation too.

Question 2

Does the power plug rotate once it is plugged into a wall socket, or is it static (stuck in its position)?

It is static (does not rotate).  But it doesn't stick out much from the wall when plugged in and the cord comes out parallel to the wall 45 degrees from "straight down" so it does not block an outlet below it.

Question 3

Just purchased this model.  The input and output for the coaxial and phone are not marked.  Which ports are the input ports?

The IN/OUT markings are hidden by the cable management bracket.  Slide the bracket off, away from the outlets, to reveal the markings.

I bought some security cameras and they "highly" recommend a surge protector 
+UL-1449 rated
+Clamping voltage of 330 or lower
+ Joule rating of at least 400
+ response time of 10 nanoseconds or less.
I don't see this (except the Joule rating) in the specs, Would anyone know if this unit meets these requirements? Thanks in advance!

Remember the Joule rating for this device is the sum of all the individual protection elements. So some nominal amount would be for the Phone Connection, some for the Coax and the remainder 1/3 for the Line to Ground, 1/3 for Line to Neutral and 1/3 for the Neutral to Ground elements. The question you need to answer first is whether you need protection after the MOV's have reached their useful service life, that is, when the MOV's are no longer working, do you want the outlets to be on without protection or off. If you need the outlets to be on, then you want to make sure that this device fails in that manner. The next question is what happens in a brownout or power outage. If you need to have continuous power, then you may want to invest in a UPS and the last question then would be how long does the security system need to stay running in the event that power is out.

This surge protector is no different than the myriad of surge protectors that use MOV's at the heart of their protection. During a surge, this protector's MOV's eventually turn on (but not right away and for the time they are not on, the very electronics you're trying to protect are getting the surge voltage & current). When the MOV's turn on, they begin diverting the surge to both the NEUTRAL and the GROUND. Hopefully there is a very low impedance path to the mains electrical GROUND because if there isn't the surge will make its way into the equipment you're trying to protect. If you need serious surge protection, not just something that diverts the surge and hopes for the best, look into Series Mode surge elimination which does not divert the surge to ground, rather slows it down in real time, stores it as energy and then dissipates a little at a time onto the NEUTRAL wire. Since it doesn't rely on an MOV it will not wear out or need to be replaced each year (something you read in the fine print of MOV protectors). You can buy them from $60 (used) to $140+ (new).

Most of my electrical devices are two prong and not three.  Will this still work?

The real question is, "why?"  A UPS provides fairly powerful surge protection/suppression by itself, mostly because it's for critical equipment (i.e. servers, workstations, your home PC, VoIP, etc) that you need uninterruptible power for.  A surge protector does just and only that... protects against spikes over 110 volts.  Surge protectors/suppressors are used for less critical devices that you just don't want fried or blown out by an electrical surge, like from a lightning strike.  I'm guessing the UPS is for a server or home PC by the way you're asking, and it should be enough.  You could use both, but it would just be more expensive... and overkill.

The response time is quick once the voltage threshold of the MOV's are reached.

The question you should be asking is where is the surge going until the MOV's threshold is reached? And secondly once the threshold is reached and the surge is diverted to both the Neutral and the Safety Ground wire, will your residence have a low impedance return path to the mains electrical panel.

The manufacturer cannot guarantee that the surge will make it's way back to the mains electrical panel so the surge will seek another path now that it's on the HOT, LINE and GROUND and the electronics you are trying to protect becomes even more of a target than when the surge was on the HOT only.

Until the voltage threshold of the MOV's are reached, the surge is going straight into your electronics and or appliances (not good).
