Does your item come with the old-style heat sink (as shown in your picture) or new heat sink version released by AMD? The old heat sink will warp after a year of use and the card will overheat and get damaged.

The heat sink is as shown. The card produces very little heat and the heat sink is of little importance. I use the card in an older server which has a bad onboard video processor. This card is very low powered compared to other cards. As such, it only really makes sense for folks that need a PCIe x1 card. If you're looking for high performance, and thus something where the heat sink is a concern, you'll want something else.

can this be used to display 1080 to 1440p video in HDR?

No. The display port adapter on this card is v1.2, and v1.4 is the minimum for HDR support.
