Mine came with a charger and cable all in a plain white (non-Apple) box.   There was a very slight dent on the rear metal case.

I did receive a cable and cube in the box with mine. I am not sure if it is a "lightning cable" or not...it is just the standard good old apple cord with the cube

A refurbished B grade unit will have minor dents and scratches due to normal wear and Tear. The product will still be fully functional. The Grade A will have minor signs of use.
