Hello. I have 4 of these cards used in various configurations. The HDMI port on this card will properly operate a standard High Definition TV, which is 1920 x 1080 resolution. The size of the screen doesn't matter, 43" or 75" is still 1920 x 1080. If your 60" TV is a 4K type, then it is 3840 x 2160 resolution. I have used the HDMI port on this card to drive a 38" Wide Screen monitor at 3840 X 1600 resolution, so I would "assume" it will drive a 3840 x 2160 resolution. I would not recommend this card for gaming.

Does this require a PCIe x16 slot, or will a PCIe x8 slot work?

I could be wrong, but I believe it needs the X16 slot. It won't fit in an x8 slot.

How can I get this to see three monitors?  I'm using VGA,DVI and HDMI to DisplayPort cbales, Dell 260 Vostro, i-5, 4gb, Windows 10 64 bit, and three Dell U2312H monitors.  It only sees the VGA and DVI connected displays.

Hello, I have 4 of these cards in various configurations and have had no problems using all 3 ports simultaneously. Since your problem is on the HDMI port, it is most likely the cable. I have several "DisplayPort to HDMI" cables and they are "NOT" reverse-able.  Carefully check the specifications of the cable, my "guess" is that it is a "DP to HDMI".  Many of them have chips in one of the connectors that make them direction specific. Some of them will have a symbol of a monitor embossed in the mold on one end to indicate which end connects to the display.
