Mine did not come with any cords. Device works as advertised though.

Question 2

Do I have to use the HDMI out to view supplied content? Or can I just use my PC?

Yes.  Unfortunately there is NO pass-thru when the PC containing this card is powered off.  So you'll need 2 PC's powered ON regardless of intent to capture or not.

The other option is capture on the same PC with both the BMD card and your main GPU on the same system.

This card does NOT support HDMI 2.0.  I was never successful in getting it to work with any output and monitor that is HDMI 2.0.
