o you have to have usb 3 ports in your computer or will it work wit usb 2 ports?

It works great with USB 2.0.  It is incredibly faster with 3.0.

Yes. The huge majority of USB drives use the USB Mass Storage standard, which is well supported in Linux, Mac, and Windows

Do I need to specify fromatting for MAC?

No.  When I plugged it into my USB port, Time Machine opened and asked if I would like to use this as my Time Machine backup, which I did.  Now anytime I plug it in Time Machine automatically backs my computer up.  I hope this helps.

Yes, there's a usb 3.0 cable included.  It's not very long; maybe 1 foot?
