Does this splitter require a power adapter?  If so, does it come with one?  Or, what should I buy for it?

Yes, it works great.  Bought one for our computer that runs the monitoring system in our compressor room here at work.  We are running two 42 inch Visio displays with it.  One in the compressor room and one in our maintenance shop.  Works very well.

What if I use a 1080p and a 720p monitor on this product? Will the 1080p monitor switch to 720p? or will the 1080p be 1080p and the 720p be 720p

It all depends on the quality being received. If you're receiving a 1080p quality image, the 1080p monitor will see 1080p and the 720p monitor will see 720p(because that's its cap). If you're receiving a 720p quality image, both monitors will have 720p quality because that's the image quality being received.

could i use this to display two identical connections on two hdtvs from my ps4?

yes you can i use it to output one signal to a capture card and another signal to output to a monitor and i dont have it powered and works just fine but my wires are also only about 2 feet long

If I use this on a computer to connect two monitors, do they show up as one or two monitors to the PC? I'm wanting to do an extended display, not show the same image on both displays.

No, this will not do what you want. Same signal/image will be on both monitors.
