If this actually come as pictured? Is it the lamp and the housing?

Yes, but I ordered mine directly from Newegg, not one of the "merchants" as I see shown here on this page now. I would be highly cautious of purchasing a replacement lamp for $65. Most of the time you'll be lucky to get 300 hours out of discount bulbs.

Would this fit a epson 6500ub projector ??

The 8350 and 6500UB use the same V13H010L49 lamp and I used this one in my 8350, so by way of A=B=C logic I'm gonna say yes.
