Does this include all 3 supports seen in the picture?

No. It only includes the keyboard support which is the one in the bottom of the picture.  The picture should be removed as it is deceiving.

I had wondered the same thing when I saw that picture but I had looked up the part number at the mfg website and saw it was only the keyboard.  For myself, I didn't care as that was all I needed as I already purchased one for the mouse at an earlier time.

It's a great keyboard support.  I have been using it for about 8-9 months with a Microsoft comfort curve keyboard.  I use a Fellowes 9180201 Gliding Palm Support for the mouse which is similar to what is shown in the upper right side of the picture except it also has a rubber flap that keeps you hand from being too far forward with respect to the mouse so it is placed in the proper position.
