Does this mount come with grommet mount hardware, or just the desk clamp?

if you can't use the desk edge clamp you can drill a hole in the desk and use the supplied hardware to bolt it through and mount it that way. My desk has no edge so that's what i did. Just make sure you correctly calculate where the screen is going to be when you mount it this way. Measure twice, drill once.

How do you rotate the monitor form horizontal to vertical?

You can simply rotate the monitor while mounted from horizontal to vertical in either direction you choose. You 'can' screw the nuts on the side to make it looser or more firm depending on the monitor's weight, but neither are meant to hold it in position permanently, so you have control over all positions of your monitor while mounted.

is there a weight Limit, or a max weight listed on the instruction?

There isn't one in the instructions but if you go to newwegg then go to the product page is where i  found out  all the info that your looking for in the specifications and it also tells you in the overview for the height. But if that doesn't work for you there is also a picture in with all the pictures that has it dimension laid out out. but if that doesn't help i have a 27" Benq and at max night it is 15 and 1\8 form the bottom of the monitor to the top of the desk and it can support up to 18 lbs monitor.

How large is the clamp? We have a thick desk we would like to use this on.

It opens to about 3 1/4 inches, should fit just about any desk. If your desk is less than 3 inch you should be good.

How tall does this stand go? I have 4 monitors 27" and was wondering if i could use this to mount my 4th above my main monitor

I have a 27 inch mounted on this and have it set about on inch from maximum, which gives me about 11 inches clearance from my desk.  I think the best you'll get is about 12 inches, which is not nearly enough, so I would look at a taller mount.
