Does this model (ER-X-SFP-US) come with a AC adapter?

Yes, it comes with everything needed to hook up and go. We only use ours for a very small isolated portion of our work network, but it has all the features we needed as well.

Can you disable PPoE?

Not sure what you mean, you only configure PPPoE if you need to use it. Its not generally something that is configured/enabled by default.

able to support 220v?

Mine came with a 120V US AC adapter, but the router itself supports "9 to 26 VDC" on a coaxial DC plug if you want to get your own AC adapter.

does this ACTUALLY throughput gigabit speed consitantly?

From my personal experience I think you're unlikely to get full Gigabit speeds, but when all my components are cooperating I've seen steady transfers at around 750mbit.
