Based on the pictures, is this is a 140mm fan that mounts to a 120mm support location? Might have to shuffle a few things around in the ole case, but I love this design, it's seems great for a CPU cooler.

Right you are. It's the same fan with a motor update, as the NF-A14 FLX fans. I've been able to fit it into places that were made for 120mm fans (with a little extra around the sides). It would be very good for a CPU cooler, even better with 2 in push-pull. I've got one of these on my NH-C14 CPU cooler and it will keep an 4790k with a very mild overclock at a max of +20c from ambient, while staying incredibly quiet.

Can this work on the NH-D15?

The NH-D15 comes with two NF-A15 PWM fans. Those would be the exact replacement. The NF-P14r is a 140mm fan as opposed to the 150mm of the NF-A15, so you may not get the same amount of airflow over your heatpipes. However, both fans use the same 120mm mounting holes and both have PWM, so it should work. I don't think you'll have any problems.
