How loud is this?

This will definitely depend on how dusty and hot where it is installed.  I have 2 of them, 1 installed in a mildly dusty and warm location - this one is VERY loud... 1 installed in a cool and low dust area and this one is as quite as the day I bought it....

? this has fiber optic ready yes or no and if yes what cards do i need and part#

Yes it is ready for fiber optics.  No it does not come with the GBIC module with the base switch.  You need to look up the correct GBIC for the type of fiber optic cabling you have.
us dlink com - products - business-solutions - 52-port-gigabit-web-smart-switch-including-4-sfp-ports - the switch specs  But you need know your fiber optic mode e.g. SX- Multimode, etc.  This is based on the type of cable and the fiber optic module you have on the other end.  Remember you only have 4 fiber ports, not all 48.  Per the specs: 
Optional SFP Transceivers
• DGS-712       1000BASE-T Copper SFP Transceiver
• DEM-310GT   1000BASE-LX Single-mode SFP Transceiver up to 10 km
• DEM-311GT    1000BASE-SX Multimode SFP Transceiver up to 550 m

I have used these switches also with port aggregation to have increased bandwidth between the two switches.

What is the power supply voltage? Does it support 220V?

We use all 110V for all of my servers and peripherals.     If my memory serves me, I don't believe this unit is capable of handling that amount of voltage and surely wouldn't try to find out.    I can tell you that I'm very pleased with my purchase!!
