Question 1

Is it possible to mount 4 of 3.5 hdd inside? I know specs tells you only 2. But the picture suggests that each of 2 brackets can hold 2 it even appears that they have 2 rows of mounting holes.

I need to preface this by saying that I recycled those two brackets a year ago when I built the system, but I'm quite certain you are correct about the second set of holes on each of the two brackets.
I think the real question is whether a mATX motherboard will clear drives mounted in the lower set of holes.
I ended up running 2.5" drives an an Icy Dock Flex-Fit Trio.

Does this require rails to mount?

Not really, it depends on how heavy your equipment is.
I am using this case as storage array, currently have 3x 3.5 HDD, 12x 2.5 SSD in the 2x 5.25 bay, 1 PSU, 1 intel raid expansion card. 
Used 3 nuts on each side with no rail, or support at the bottom, I have 4 post rack.  it holds pretty good with 3 nuts each side!

can it fit any sort of graphics card (for setting it up and stuff)?

You can fit a low profile graphics card in most 2U cases including this one. If all you need is a card to set up your system, a full size card will fit without the top on.

Question 4

Does this come with fans? If not, what size of fan should I get?

It does come with 2 80mm fans that are quite loud and not very efficient at pushing air about. I went and purchased 2 replacement fans costing $20 each that performed virtually the same. You cannot fit 120mm fans in this chassis if that's what you were hoping for. So to answer your question, yes it does, and to add to your question, they probably work just as good as any replacements you purchase.

can this support ATX Motherboards?

In order to fit a standard ATX you would need an external power supply, since this size board is too wide, and at best you would only have two slots available. If you need a 2U rackmount that supports a standard ATX then I would take a look at the iStarUSA D-200 instead.

Question 6

I know description says ATX PS2 PSU, but can any other PSU fit inside it?

I mean, anything smaller will fit, I have an sfx psu in(the psu came with a bracket), but, without a bracket/adapter you'd have a hard time mounting it. Unless you build one yourself or use double sided tape or something

Can you fit a dual slot card inside of it?

Nope, it is only 1U, it might fit side ways though....

If your card is half width dual slot, then it will fit.
