How does this attached to the back of the monitor? There are not any screw holes in the back of my monitor.

I don't think you will be able to use this product then. On both arms there are two holes in each of the four ends of the 4 point star type frame. You can use 4 inner holes or the 4 outer holes. If your monitor doesn't have holes in the back of it, look for some type of screws or filler material that might be covering the holes that you can mount the monitor with. If it doesn't have those...then you might truly not be able to use this product. I am completely satisfied with this product by the way, and it is exactly what I wanted for my home set up.

Does this stand requires to be screwed on to the desk??

No, it has the option of being screwed to the desk, but also comes with a baseplate as well.
