how does "stick" to the car? How do you remove it?

The suction cup has a sticky material on it. To remove it, release the suction, lift the tab and pull as hard as you can. It took quite a bit of effort to take it off, but that really just re-enforces the belief that this is very good quality. The sticky stuff left no residue on the car.
Truthfully, I'm almost tempted to say that without suction, the sticky material is strong enough to hold it on your dashboard.

Does this work when you have a case for you iphone?

Yes, I am using iPhone 5S with protecting case and it works just fine and better than phone alone.

I had this holder for my S4 and upgraded to a Note 4 and unfortunately it doesn't fit my Note 4. I'd assume the Note 3 is the same size as the Note 4, so I'd say this probably doesn't fit a Note 3 either. It's sad because it's a great holder and grips to almost any surface!

I am using this device with my Apple iPhone 4 S. You didn't mention if it was the 4 or the 4 S but I assume you meant just the iPhone 4. I don't think it would have a problem because it actually expands sideways to except different size phones. It is fairly cheap, so not a big loss if it doesn't work. Good luck.

What is the diameter of the suction cup or the part that attaches to a surface?

2.5" diameter.   It does need a fairly flat surface, as opposed to smooth, to work.  It does stick quite well under those conditions.   I have not tried this on a concave surface, but on a convex (arching out), it is not useful at all.  I do recommend this item, though.  It is very stable and easy too use.  rrh

It easily opens 1/2" or more bigger than my Nexus 4, so check the specs to make sure, but i believe so.
