It comes with the casters off.  You have to install them if you want... so yes.

I was thinking of using this for audio equipment.  Are there any shelves that can be purchased?

This is a 19in. rack. it's a standard.
Many manufacturers make shelves and accessories for 19in. racks (Hammond, Rack Solutions, even iStar sells some on their site.)
You will need to be careful about what you want to put on the shelves. These racks are designed for equipment that hangs off the rails in the front. The selves do take up some side-to-side room, so big components like power amps, preamps, receivers might not fit on a shelf- but they might have rack mounting options that will work.

Does the height represent the entire height including castors or just the box itself?

The unit is 24 inches in height with the casters. Without the casters, it's approximately 20.75 inches.

does this rack have a locking mechanism?

There is a wheel lock on one side (see left wheel in 1st picture), if that is what you mean. The shelves bolt in.

Does this rack include the captive nuts and screws for the equipment? If not, where do I buy those items?

Yes, ours came with about 30 captive nuts and stainless screws.
