does it boost the volume and do you plug like headphones into it. for a louder sound?

It doesn't boost the volume, you would need a headphone amp for that. This just lets you turn the volume down. I bought one because I use my headphones with my Playstation, but it's too loud most of the time and there isn't any volume control, so this allows me to lower it to a manageable level.

Hi,
At the top of the volume control, is that a 3.5mm input?  I'm trying to  picture the flow of this unit and I assume a male 3.5mm gets plugged in to the top of the unit, you adjust the volume, the the output goes through the cable into the 3.5mm male plug that is attached to the unit.
Is that correct?

Yep, you've got it.  It's simply a link that goes between the device and the original 3.5mm headset  to give you the ability to have a physical volume control for your headset.  3.5mm plug goes in the top of the Koss device, and the 3.5mm connection of the device goes into whatever audio device you want.

I used mine to control the volume of my head phones which were connected to the back of my tv while I was on 3rd shift.  The tv wouldn't control the volume of the headphones for some reason so I had to have something to do it for me.  This did the trick.
