Can you confirm the installed processor is a 4th Gen Intel® Core™ i5-4570? My research on this model shows that processor matches the specs listed, but would like to be sure since they don't list the specific processor.

Yes, as stated in the overview that is the processor included in this PC, all specs match up exactly as described a 4570 with 8 gigs of ram and win 10 pro pre-installed on a 500gb HDD. I'd jump on it ASAP if I were you, price won't last long (well at least if until it goes on sale again).

How many pci-express slots (and what size?) are on the motherboard?

1st search result: on psref.lenovo one page: "M83%20Tower.pdf" slots 1 and 3 hold "x16" length cards, 75 Watts or less. Slot 2 "x1" length.
Common for this "Q85 Express" chipset. 2nd of the two pictures above show full-height cards fit.

Question 3

Does this have hdmi or displayport... The picture looks like it has 2 DP. 
The specifications for both are null..?

The system has two DP without any other video port.  You can buy a DP to HDMI cable to connect to the monitor and both DP worked.
