I realize this can be used for either hand, but if you are right handed, is the left click under your thumb on the left side of the mouse?

Eight click and left click can be set up in "windows preferences". You can tell windows that you want the mouse configured for a lefty or a righty.  But to directly answer your question....... yes, if you are right handed, the left click is on the left side under your thumb. (The windows defeat mouse set up is the bright handed set up).

Could someone please explain to me how this is better than the thumb controlled trackball?

I play WoW, and I use no macros/healbot/addons, and I have 4 lvl 90 healing toons.

Just wanted to get some input.

Well, it's been 6 years since this question was asked, so I doubt the OP will see this AND knows it by now, but here goes. I avoid thumb-only trackballs because, well, it only uses the thumb! And, in my experience, anything that uses ONLY ONE FINGER makes that finger get pretty tired! I mean, if you've been using one for years, that finger will be pretty strong and accustomed to all that activity, but switching from a trackball that you can access with all five fingers AND the palm of your hand to a thumb only device could have serious cramping issues. Plus, I'm sure, it doesn't help with carpal tunnel syndrome type problems (the same motions over and over can cause problems) To each their own, but buyer-beware!

I have windows 8.1  on an asus rog g75vw laptop and it works perfectly, just plug the mouse in a usb port, (I have usb 3.0), and windows instantly recognizes it.

I suggest not to buy this product due to it not having a scroll wheel. I was very disappointed with myself for not noticing when I purchased it. There are trackball mice that use a scroll ring to substitute  for a scroll wheel. There are several models available on New Egg.
